We work with PCTs and health services as a core part of many of our commissions that cross health, social care and often housing. The focus is mainly on primary and community based health care services such as intermediate care, out of hours services, mental health services, therapy services, equipment and adaptations services. Work we undertake around the management in the community of people with long-term chronic conditions can directly impact on service and care pathways, and on levels of hospital admissions and delayed transfers from hospital into community settings.

New services such as extra care housing can provide a focus for community outreach services on a 24 hour, seven days a week basis to support people with long-term conditions, as well as day services within the schemes themselves.

In work around integrated teams we look at workforce issues and the potential role of housing alongside social care and primary care staff.

PCTs are increasingly recognizing the importance of suitable housing and housing services in taking pressure off the hospital system and to delivering patient choice through community based solutions.
